
Sims do not develop conditions automatically. Instead, they can be assigned disorders through various means such as random auto-assignment within the active household, manual selection, randomization via cheat menu, age-related progression, or through psychiatric evaluations. When visiting a psychiatrist, Sims have a small chance of receiving a diagnosis.
Each Sim is given a neurotype—either neurotypical or neurodivergent—as well as a self-esteem level that can be low, average, or high. These factors influence their chances of developing certain conditions and how they respond to life events. For example, neurodivergent infants and toddlers may experience developmental delays that can evolve into learning disabilities or neurological disorders as they grow. During adolescence and adulthood, Sims may be more susceptible to anxiety, mood, or personality disorders, with self-esteem and life experiences playing significant roles. Some conditions can increase the risk of developing others over time.
Once diagnosed, Sims can pursue treatment through various forms of therapy such as speech or behavioral therapy, psychotherapy, or individual coaching. They can also try alternative approaches like meditation, self-help, or support groups. While medication is also available, details for that are provided separately by the creator.
Mental health conditions impact gameplay by affecting Sims’ emotions, relationships, and skills. Sims may struggle socially, experience mood shifts, or form deeper bonds with others over shared experiences. The mod also includes unique social interactions related to empathy, support, and tension.
The system is modular, so players can customize their experience by installing only the specific disorder modules they want to include in their game.
